ABOUT PLASTIC SURGERY

CLEFT LIP SURGERY

This is done to correct the birth defect in the upper lip.  After surgery patient should be kept quiet for five to seven days with phenargan syrup given four hours.

Only liquid diet should be given with spoon-avoiding sucking.  Dressing should not be allowed to get wet.  it should be kept dry, Dressing should not be removed.   It will come off by itself after five to eight days after which the surgical area can be gently cleaned with soap water.  All stitches will be absorbed and they should not be removed or cut off.

Patients must take antibiotics for five days after surgery.  After initial surgery second surgery is sometimes necessary to correct minor defects by retouch which is done one year later.  In bilateral clefts only one side is repaired and the second side will be repaired one year later.

Nose will be corrected a few years later.

SQUINT SURGERY

Is done to correct abnormal position of either one or both eyes.  There are 2 muscles in each eye which can be operated upon either at the same time or at different times depending upon the degree of squint.  After both the muscles are operated upon, if there is still more squint then the muscles in the opposite eye can be operated.   So there are 4 muscles to be operated which can be done at 2 different times or 4 different times if only one muscle is operated each time.

  After surgery eye may swell  & may become red and it should not be rubbed.   Do not apply water.  You can foment it with warm towel 10 min twice a day.

Use the eye drops three times and eye ointment at bed time for three weeks. (Polisporin eye ointment or drop.)

It is important to start wearing glasses right after surgery (If you wear glasses).

You have to start eye exercise from second day to make the operated eye muscle stronger.   Exercises must be done hundreds of time every day for 3 months or elese you wont get full benefit of surgery.  Eye surgery is only one step in correcting squint other two important being post operative eye exercises and wearing glasses if corrective glasses are necessary.

  Use sun glasses to prevent light sensitivity. You can resume your job or reading or studying next day, it will not cause any harm.  Any stitches used for surgery will be absorbed in 3 to 6 weeks. They do not have to be removed.

Squint surgery has no effect on your vision - it will neither improve nor deteriorate.   It will be the same as before.

EAR SURGERY

Is done to correct external ear defect like protruding ears or lop ears etc.

The Dressing should be removed after 10 days stitches will be absorbed  in 3 to 6 weeks.

After dressing is removed patient should wear a ear cap at sleeping time for 3 months to prevent rolling of ears.  Final shape will be seen after 6 months.

NOSE SURGERY

Done to correct shape of nose.  Packing in nose should be removed after 4 to 5 days and dressing after 8 days. It takes 6 months to a year to see the final shape of the nose.  You should protect nose after surgery from any kind of injury for the rest of your life.

After surgery patient is prescribed Vitamin C 500mg twice a day and Actifed 1 tablet twice a day for the longer duration or as and when you have cold to prevent allergic rhinitis, colds etc.

EAR LOBES REPAIRS

Do not get wet, do not remove dressing till it falls off.  Sutures will be absorbed after 3 to 6 weeks.

Ear should not be pierced for 6 weeks. New holes should not be done in repaired area.   You must not use heavy ear rings and they should be removed when sleeping. Ideal is clip-on type.

LARGE GIANT NEVUS

Staged excision is done upto certain limit.
Final operation is to be done by skin graft.
Some times a very large giant nevus can never be completely removed.  There is 9 to 15% change for it too turn into cancer any time.

You should consult an oncologist for possible chemotherapy or any other kind of cancer treatment.

TCA CHEMICAL PEEL

This is done to improve dark pigmentation of facial skin or to remove fine lines on face.

After treatment face will be swollen and red.  It can be treated with cold ice bag applied to face for 5 to 10 min. at time.  The treated skin will form dark scales and will start falling off from third to fifth day after which you can wash the face with cold water and apply cold cream.  You must avoid direct sunlight for 10 to 12 weeks or else the skin may become dark.1

Apply mellalite cream to face daily twice, once in the morning and once before going to sleep.  This will prevent skin from getting dark.

TCA peel can be repeated after 6 months if necessary.

INTRODUCTION FOR DAB

DAB- Dermabrasion is done to improve scar or pigmentation.  It only improve the scar and cannot completely remove the scar.  The scar result depends upon the patients response to healing and proper post operative care by the patient.

After DAB skins will look red and by 2nd day it will be covered with a Scab.  This scab will fall off by itself after 8 to 10 days.  Till that time you must not allow it to get wet.  Keep it dry. When the scab falls off new skin will be red and tender like Babes skin.  It should be protected from direct sunlight for 12 weeks.   Also it should be covered every morning and night with Mellalite cream which will prevent it from getting dark.  You can wash it after scab comes off.

Healing will continue for 4 to 6 months by which time you will see the final result.   If the result is acceptable nothing further should be done.  If this was only a test DAB - and if the result is good - the entire scar can be treated with dab during the next camp. If no improvement is seen after test DAB - You are not a suitable patient for further DAB and your scar will have to be surgically removed.  Whatever treatment is chosen no scar can be completely removed by any kind of surgery.  It can only be improved sometimes it can be camaouflaged with cover up or make up.

PTOSIS

Surgery is done to correct a droppy or dropped eyelid.

The dressing will comeout by itself and should not be removed.  The Dark stitch can be removed by pulling out after dressing comes out.  Eye ointment should (Polisporin) be applied 4 to 5 times and at night before sleep.  If eyes remain open during sleep (as observed by relatives or parents) it should be patched with an eye pad every night for the rest of your life.

Sometimes surgery will open the eyes partially but enough to make you see.  This is because the muscle is too weak and cannot be improved further (a non functional underdeveloped or paralysed muscle).

There are many deformities which can only be corrected partially, and some deformities cannot be corrected at all by any kind of surgery.  One has to accept and be happy with whatever improvement has been achieved.
