1) If you are considered then you will be operated. upon on: the mentioned date.
    (A) Please bring HB and Urine test report if selected for operation.
    (B) Operation will be cancelled if patient does not report by 8.00 AM on day of operation with empty stomach.

2) Spectacle Number to be brought with you.

3) You cannot be considered for operation for the reason ticked below.

4) Child is too small to be selected for operation -Try next year.

5) You should get operated upon only in well equiped hospital like Medical College etc. It is not possible in the camp as it is too ;lengthy.

6) Wait untill next camp for decision.

7) Use spectacle ( Glasses ) .

8) Use pressure garments.

9) Consult orthopedic surgeon. .

10) Consult Optholmoligist.

11) Consult Neurologist.

12) Consult ENT Specialist.

13) Consult Skin Specialist.

14) Consult Dentist or Dental College.

15) Consult Speech Therapist.

Note: -Organisers reserve the right to cancel the operation without assigning any reasoneven if you are selected.
